Renewal Evaluation Form (Word) (PDF)
Each of the following groups fills out a separate renewal
evaluation form:
- Minister (self evaluation)
- Governing Board, or Supervisory Board, or Supervisor
- Committee on Ministry
Professional Development Agreement (Word) (PDF)
The Professional Development Agreement only needs to be filled out by the minister and attached to the self-evaluation.
Statement
of Affiliation (Word) (PDF)
To be filled out by Community Ministers.
Waiver Request Form (Word) (PDF)
Use if you are applying for a waiver of MFC rules and policies.
