How to Take Advantage of the UUA Health Plan
- Read this document carefully and review the benefit details to see which plan is right for you.
- Check the Highmark Blue Cross Blue Shield website to confirm which of your healthcare providers and others you might like to select are in the network. Go to Highmark BCBS, click on one of the provider categories in the "Find Providers" box, and use the “Advanced Search”. In Step 3, choose “BCBS PPO” as our product type. If you have any difficulty, contact the Unitarian Universalist Association (UUA) Health Plan Coordinator in the Health Plan office.
- Review the information on this website for information about your individual premiums.
- Complete the Enrollment Application. Our step-by-step online enrollment takes just a few minutes.
- To request an Enrollment Application to be sent by mail or email, send an email to the Health Plan Coordinator.
- Call the UUA Health Plan office at (617) 948-6405 for answers to specific questions. You may also visit our Frequently Asked Questions page and find the information you're looking for.
