UUA Group Insurance: Dental, Term Life and Long-Term Disability

The Unitarian Universalist Association’s (UUA’s) Group Insurance Plan (UUGIP) offers Long-Term Disability Insurance (LTD), Life Insurance and Dental Insurance coverage.

Eligibility

An employee must work at least half time to be eligible for the UUGIP benefits.

Enrollment Timeframe

Employees should enroll in UUGIP within 60 days from their date of hire.

If an employee neglected to sign up for coverage when it was offered within the intial 60 day time period, they must provide evidence of good health at the participant’s expense by completing a statement of health form for LTD and Life coverage and will have waiting period for certain procedures for dental coverage. If an employee is not offered the insurance within their first 60 days, please contact the UUGIP office.

Enrollment Procedures

Participants should contact the UUGIP Administrator for an enrollment form. Please contact the UUGIP Administrator (groupinsurance @ uua.org).

Premium Costs

Premiums are determined in the same way for all participants, regardless of age and occupation.

  • LTD Premium: 1% of salary (or salary + housing for ministers)
    • Example: If Rev. Jones has a salary of $10,000 and a housing allowance of $15,000, her LTD monthly premium would be $20.83.
      (Salary + Housing) X 1% = $25,000 X .01 = 250
      250/12 Months = $20.83
       
  • Life Premium: $0.32 for every $1,000 of benefit. Benefit is two times salary (or salary + housing for ministers) up to a maxium of $150,000.
    • Example: If Rev. Jones has a salary of $10,000 and a housing allowance of $15,000, her Life monthly premium would be $16.00.
      (Salary + Housing) X 2 = $25,000 X 2= $50,000
      $50,000/$1,000 = 50
      50 * $0.32 = $16.00
       
  • Dental Premium: Fixed rate of $41.90 for individuals and $101.60 for families of any size.
    • Example: If Rev. Jones has an individual dental plan, her premium is $41.90. If she adds one member of her family or five, her premium would be $101.60.

Premiums Payments

Premiums can be paid by congregations, participants, or a combination of the two. This decision is made at the congregational level.

They can also be paid on a pre- and post- tax basis. Typically, LTD is paid on a post-tax basis. Life and Dental can be paid pre-tax.

Changes to Participant Information

Salary changes, terminations, changes to address and other changes to participant information should be e-mailed to the UUGIP Administrator as soon as they are known. Please be sure to let the UUGIP Administrator know whether or not the terminated employee will want COBRA coverage.

Billing

Individual participants and congregations are billed monthly for the previous month’s coverage. For example, in February, the bills will be for January coverage.
